Benefits: Supplemental Life Insurance

In addition to the current Life and Health Benefit Program, the Conference established a Supplemental Life Insurance Plan and a Dependent Group Life Insurance Plan. Since this coverage is offered on a group basis, the cost is much lower than equivalent insurance purchased on an individual basis.

Supplemental Life

For the Supplemental Life Plan, there are five options to choose from:

• 1) $10,000   2) $20,000   3) $30,000   4) $40,000   5) $50,000

The cost to each participant is listed below:

• Example:
• Age: 43
• Life Amount: $30,000
• Monthly Cost: 3 x $10,000 units x $1.50 = $4.50

Only participants under age 65 are eligible for this Supplemental Life benefit.

1. During the open enrollment period, you may increase your coverage by $10,000, or if not enrolled, may elect coverage of $10,000. If you are retired and under age 65, the only change you may make is to cancel current coverage.
2. For ease of administration, premiums are automatically administered through payroll deductions. Please inform your local treasurer of any change you make.
3. To enroll or make a change, complete sections B, C, E (only if you are enrolling in dependent life) and H on the application form enclosed and return to the Conference Service Center no later than November 30, 2006, to be effective January 1, 2007.

   * Note – In section C, mark “Change Coverage Amount” and indicate how much supplemental life you currently have and how you want it changed.

Dependent Group Life

The Dependent Group Life benefit offered will provide you with $5,000 in the event of death of your spouse and $2,000 in the event of death of any of your eligible children. The cost for this benefit is $4.08 per month regardless of the number of total dependents that you my have.
