Information on all of our Health Benefits, ID Cards, Claim Forms, etc. can be obtained either by phone or on the web.

By Phone

Medical, Pharmacy, Mental Health, Vision

Call 1.866.804.0976 and choose the appropriate number at the prompt to connect to:

• 1 for Blue Cross Blue Shield - Medical Plan
• 4 for United Behavioral Health (Active Only) - Mental Health and Employee Assistance Program (direct phone 1.800.788.5614)
• 5 for Medco - Pharmacy Plan (direct phone 1.800.841.2806)
• 6 for Optum NurseLine (help with general medical information or needs)
• 7 for Vision Service Plan (VSP) - Vision Plan (direct phone 1.800.877.7195)

You will need your group number for Medical and Pharmacy. Your group number is listed on your ID card. You will also need your ID Number, which is also listed on your ID card. For Pharmacy, the ID number is the last 9 numbers as listed on your card.

Dental

Call 1.800.244.6224 to connect with Cigna, our dental plan provider. Our Dental Group Number is 2464058. Your ID Number is your Social Security Number. There is no ID Card for Dental; however, you can print this information and give it to your Dentist.

On The Web

The general Website for the HealthFlex program for North Indiana is www.gbophb.org. Choose the HealthFlex/WebMD button. You will need to register as a new user (there are instructions on the site to assist you). Once registered, you will be able to enter the HealthFlex/WebMD site and have access to all of your health information, plans, forms and products. You will see a menu option that will allow you to choose all of our health plans and be directed to their sites. After you choose one of the vendor sites, you will need to register for a unique user name and password. Keep a record of these for future use.

You may access all of the plans directly, once registered, by going directly to their websites at the addresses below. It is our recommendation that you enter the sites through www.gbophb.org so that you may benefit from the HealthFlex/WebMD site as well. This is a personal page designed for you. You may customize the information for your needs.
